How to register for St Louis Home Care
1. By telephone
You can select St Louis as you aged care provider by either phoning us directly on 08 8552 1481
Or selecting us as your My Aged Care provider by calling 1800 200 422 or head to www.myagedcare.gov.au
2. Provide some basic information
You'll need to provide some information including your Medicare number (consent required if contacting us on someone else's behalf), your full name, date of birth, and residential address, a contact number and pension number.
3. Answer a few questions to help identify your needs
To help us determine the type of service you need, we will require you to answer a few questions about your daily life and elaborate on your daily needs. We will provide you with a referral number should you need to refer to it in the future.
4. Confirm your St Louis choice with My Aged Care
If you haven't done so already, please contact My Aged Care (1800 200 422 or www.myagedcare.gov.au) to let them know St Louis Home care is you preferred provider.
Note: You must be registered with My Aged Care to access most government subsidised aged care services.
